I am about to submit a Form 1 to SBR a Glock. It is chambered in .40 S&W. In the box for caliber, does the ATF prefer just ".40" or ".40 S&W"? Or does it make no difference at all?

I am sorry to do this but I have another stupid question I can not find an answer to. On my Form 1 to SBR a Glock, in the box for the original manufacturer and location, should I put Glock's address in Austria or Smyrna, GA? the frame says "Made in Austria", but then right below it says "Glock Inc. Smyrna GA". Can anyone help? I want to get this done right so they do not kick it back to me as an error. Thanks for all your help in advance.

I was hoping to find out before I drop by my forms off with the Sheriff of Tarrant County tomorrow. I guess I will gamble and fill out the form with the address for Austria.
It's made in Austria and imported by Glock in Georgia...
